Step One

Complete the referral form by clicking on the link below. Instructions for sending the completed form are in Step Two. 

Step Two

Contact us and send the completed referral form by fax or email to begin the intake process.

Step Three

Provide us with copies of the required documentation to determine eligibility. 

• Current school IEP

• Developmental evaluations with a diagnosis of Autism

• A photo copy of your health insurance card

Step Four

Once all required documentation is returned and eligibility is determined, a team member will contact you to complete the remainder of the intake process and initial assessment.
